According to the World Health Organization (WHO) in 2018, stomach cancer (UTDD) is a common cancer, after only liver cancer and lung cancer in frequency, accounting for 10% of all types of cancer.

In Vietnam, there are 17,500 new cases of stomach cancer each year and 15,000 deaths (86%), the main cause of death is late detection. If detected early and treated promptly the survival rate after 5 years to 90%.

Currently, the average survival rate after 5 years of detecting stomach cancer in countries is very different: USA 29%, Vietnam 14%, Japan 80%. How has Japan done that high five-year survival rate of stomach cancer? The answer is that there should be a backup program with the goal of early detection and timely treatment.

According to the Japan Cancer Society, the gastric cancer screening program includes

  1. Prevention: Includes changes to the diet, avoidance of exposure to carcinogens, taking medications etc. to minimize carcinogenic factors and strengthen immune factors against cancer.
  2. Screening: Targets for detection of prectero cancer lesions such as Adenoma, dysenterous, intestinal malforming and early cancer, with timely treatment.

1. How to prevent stomach cancer?

According to the Japan Cancer Society, gastric cancer prevention consists of the following main points:

Reasonable physical activity, strengthening health, avoiding obesity

Maintain mental health, avoid stress

Strengthen antioxidants: Eat more greens, fresh fruits, provide enough vitamin C, vitamin E …

Do not smoke, avoid abuse of alcohol, smoked meat, fried, eat a lot of red meat

Do not eat salty: According to who, an adult should not eat more than 5 grams of salt per day (2 teaspoons). Reduce salt intake in the following ways:

  • Regularly eat boiled dishes.
  • Limit processed foods.
  • Reduce stock, rim, do not eat main noodles.
  • Read the salt content on the product before buying.
  • Do not sprinkle fish sauce, meat juice, braised fish into rice.
  • Do not drink bowls of pho water, vermicelli, soup.

HP bacteria: As recommended by WHO, HP bacteria are classified as group I agents that cause stomach cancer.

Avoid direct contact, ingestion of harmful chemicals: Herbicides, pesticides, growth stimulants, genetically modified foods … heavy metals, radioactive substances.

Drug proculsification: Currently in the study there are a number of antioxidants, anti-inflammatory drugs (such as NSAIDs that work on Cox2) that have a proculative effect on stomach cancer but have not been widely agreed to use, used only in some special cases and prescribed by the doctor.

Table salt

It is recommended to limit the amount of salt eaten during the day

2. Gastric cancer screening

You have passed the age of 40 regardless of whether the male or female should go for gastriccancer screening, including the following steps: If you have symptoms: Superior abdominal pain, nausea, loss of appetite, weight loss … it is necessary to see a specialist in digestion for diagnostic and treatment advice. If you are completely asymptoatic, screen for the following steps:

Step 1:Identify risk factors.

Pepsinogen I/II Blood Test<3 và có hiện diện vi khuẩn HP: HP Ab (+), Xét nhiệm phân HP Ag (+) hoặc test thở UBT (+).

  • X-ray of the opposite stomach detects lesions in the stomach: Ulcers, polyps, inflammation …
  • Families with people of the same blood have cancer, have a mut mutation gene that causes cancer.
  • I have cancer elsewhere: Breast, uterus, liver cancer …

Step 2:Conduct gastric endoscopy for people with at least 1 risk above.

Step 3:Conduct a biopsy as a hist pathology for people with suspected cancer or pre-cancer lesions: Polyps, intestinal heterotrophs, atrophy … for a definitive diagnosis.

Step 4:How to follow up next? When is the next laparoscopy?

  • TEST HP Ag, HP Ab, Gastroesophageal imaging once a year.
  • People with normal gastric endoscopy, without risk factors, have gastric endoscopy every 5 years.
  • Some family members have cancer and normal gastroscopy, gastroscopy every 3 years.
  • People with intestinal malforming, severe endoscopic atrophy once a year.
  • People with laparoscopic low-level dysscopic dysscopic dysacopy every 6 months to 1 year.
  • People with high-altitude dysposm, need to remove endoscopic resection lesions, endoscopy re-examination after 3 months.
  • Early cancer is localized only in the mucous layer with endoscopic (Endoscopic – Submucosal – Dissection – ESD) in design.
  • Late-stage cancer: Depending on the stage, it will apply methods such as surgery, chemotherapy, radiotherapy …. This is prescribed by a specialist.

What to do when examining varicose veins

People over the age of 40 should have a screening for stomach cancer periodically to ensure their health

3. How to treat stomach cancer early?

According to the guidelines of the Japan Endoscopic Society, the crux of the problem must be early detection and minimal treatment.

What is early detection? It is detected pre-cancerous lesions and cancer lesions but is local only in the gastric mucosa, which has not yet invaded the lower mucosal layer.

What is the minimum treatment? It is a treatment that completely eliminates cancer damage through endoscopy and still preserves the stomach (not surgery). Currently, the EMR (Endoscopic Mucosal Resection) and ESD (Endoscopic Submucosal Dissection) method is used to cut the mucous membranes with cancerous lesions from the body.

After ESD or EMR, the endoscopy will be periodically checked on a case-by-case examination, the doctor will have specific advice but usually from 3-6 months / year first. If treated thoroughly, the patient will not need surgery, no chemotherapy, no radiotherapy.
