The role of Progestogen in the prevention of premature birth

Premature birth is a condition that occurs when the baby is born before the 37th week of pregnancy. Premature babies often have more health problems than those born full months (born between weeks 39 – 40 of pregnancy). According to the results of many studies, progestogen can help prevent premature birth for some women.

1. What is premature birth?

Preterm birth is a condition that occurs when the baby is born before the 37th week of pregnancy. About 70% of premature births occur naturally, the remaining 30% of preterm births can be caused by medical decisions or as a result of mage or fetalconditions(pre-production, forward placenta, delayed pregnancy in the uterus,…).

Preterm birth is the leading cause of neurological disability and infant mortality. Factors that increase the risk of preterm birth include: previous history of preterm birth, vaginal bleeding, multiple pregnancies, low body mass index (BMI), genital tract infections,…

2. Effects of Progestogen in the Prevention of Premature Birth

What is Progesterone

Progesterone helps the uterus grow and reduces contractions, helping to reduce the risk of miscarriage in the early stages

Progestogen is one of the 5 types of steroid hormones. Progestogens are divided into 2 groups, the progestin and progesterone groups.

Progesterone is a hormone produced by the body, which plays an important role throughout pregnancy. In the early stages, progesterone helps the uterus to develop and reduce contractions, helping to reduce the risk of miscarriage (since if a contraction of the uterus occurs in the early stages of pregnancy can lead to miscarriage – the condition of pregnancy dies in the uterus before 20 weeks).

For the later stages of pregnancy, progesterone helps the mammary glands to develop to be ready for lacts and helps the lungs work better to provide more oxygen for fetal development. At the same time, progesterone also regulates immune responses by reducing the production of pre-inflammatory cytokines and smooth muscle dilation, thereby blocking the activity of oxytocin and inhibiting the formation of a reaction that triggers smooth muscle contraction of the uterus. In particular, progesterone also prevents the self-destruction of fetal membrane cells.

There are 2 forms of progesterone use in the prevention of premature birth:

Progesterone vaginal setting

Reduces the risk of premature birth in the case of women who are pregnant with a single pregnancy and have a short cervical. The short cervical canal means that the length of the cervical canal is shorter than usual (<20mm), dẫn đến hiện tượng cổ tử cung mở sớm hơn, trước khi thai nhi sẵn sàng chào đời. The doctor may prescribe maternity use of progesterone to place the vagina gel, bullet form or capsule.

The advantage of vaginal progesterone is high bioability on the uterus because of its effect on the uterus before the first conversion in the liver. However, vaginal progesterone can also cause annoying vaginal irritation, but less overall side effects. And since vaginal progesterone has a half-life of about 13 hours, it is usually placed every day, dosage 90 mg – 400 mg;

Progestogen plays an important role in preventing singly preterm birth in 2 groups of women: women with a history of preterm birth, single pregnancy and single pregnant women with short cervical

Progesterone Injection

Reducing the risk of preterm birth in the case of single pregnancy with a history of preterm birth, this type of progesterone has a composition of 17 alpha-hydroxyprogesterone caproate (also known as 17P). The doctor usually prescribes the use of injectable progesterone for pregnant women starting from weeks 16 – 24 and until week 36 of pregnancy. The vaccination drug is Makena (a specialty) or compound blend (used for cases of allergies to the composition of Makena). When injecting the drug, the pregnant woman may experience discomfort at the injection site.

Note: Progestogen treatment does not apply in the case of multiple pregnancies, but only applies to single pregnancies.

Premature birth is the main cause of permantal death and other conditions. Progestogen plays an important role in the prevention of singly preterm birth in 2 groups of women: women with a history of preterm birth, single pregnancy and single pregnant women with a short cervical. When taking Progestogen for the prevention of premature birth, the pregnant woman should follow the doctor's prescribed.
