Treatments for hepatic and renal syndrome are mainly the use of covascular drugs, albumin and some invasive techniques, the purpose of which is to prolong the life of the person.

1. Hepatic and renal syndrome prevention treatment

  • Take antibiotics and albumin infusions for patients with cirrhosis of the liver with abdominal epidemic infections.
  • Accretion of the volume of retress for cases of fluid loss such as diarrhea, digestive hemorrhage.
  • Avoid excessive use of weeds that cause epidemic loss.
  • Do not use renally toxic drugs such as NSAIDs, aminoglycosid antibiotics.

2. Treatment of type 1 renal liver syndrome

Diagnosis of type 1 renal liver syndrome: Rapid progressive renal failure is evaluated by the patient's 2-week serum creatinine levels doubling from the original or increasing to 221 μmol/L.

The initial treatment for type 1 renal liver syndrome is the use of cosing drugs and albumin. Recommended drugs for use include: Terlipressin is the most widely used vascular drug; Midorin, octreotid and dopamine require additional clinical evaluations, where dopamine may take low doses of 1.5 – 2 mg/kg/hour in combination with albumin infusion.

Patients using the drug as directed by their doctor

Improve insymedness by using terlipressin in combination with albumin according to the following principles:

  • Terlipressin (0.5 – 2 mg per 4 hours of slow intravenous injection): Terlipressin initial dose 0.5mg injection every 4 hours. If after 3 days of treatment, creatinine does not decrease, gradually increasing the dose every 3 days 1 mg, or 1.5mg or 2 mg every 4 hours until creatinine decreases < 133mcmol/l.
  • Albumin (1g/day on the first day and then 20-40g/day).

Discontinuation of treatment if the following cases occur:

  • Creatinine does not decrease by at least 50% after 7 days of high-dose or non-reduced terlipressin use after the first 3 days of treatment.
  • Appearance of side effects of the drug: Contractions lead to anemia and arrhythmias.

In patients with a response, treatment lasts until hepatic syndrome is reversed or up to 14 days.

In addition, for patients with type 1 renal liver syndrome who do not meet the vascular drugs, TIPS is used as an alternative treatment. Liver transplantation is the only measure that brings long-term life chances to patients.

3. Treatment of type 2 renal liver syndrome

Diagnosis of type 2 renal liver syndrome: Slower progressive renal failure (serum creatinine at about 178 μmol/L) in combination with recurrent ascites or diuremic ascites.

Currently, the treatment regimen for the use of vascular drugs for patients with type 2 renal liver syndrome has not been determined. TIPS are used but there is no research to determine the effectiveness.

Doctor

Patients need to go to the hospital to be treated according to the regimen

TIPS is a surgical procedure that creates a door-to-door connection between a liver vein and a branch of the door vein with a pass through the liver. The purpose of using TIPS in the treatment of renal liver syndrome is to help reduce the pressure of the aortic vein, improve the level of dialysis and reduce the response of the covascular hormone.

Liver transplantation is the only radical treatment.
