Vascular node for varicose veins

Varicocele varicose veins are common and common in men of adolescent age and older. Varicose veins treatment with vascular node method is currently widely applied thanks to high efficiency and limiting the level of invasiveness.

1. Overview of varicose veins

Varicose veins are a condition in which the vein weakens or loses the vein valve, making it impossible for the blood to flow to the general circulation system and stagnation in the testes below. Leads to poor metabolism, increased temperature in the testicles leads to impaired sperm function, erectile dysfunction even causes infertility.

2. Risk subjects

According to many studies, about 15-17% of men in adulthood have varicose veins. Of these, 90% of cases are present in the left testicle and about 10% are on both sides. Varicose veins rarely appear before puberty when only 1% of boys under the age of 10 have it. In adolescence, the incidence of the disease accounts for 8-10%. Statistics from adolescents and above alone show that the incidence of illness increases to about 15% of the male community.

Varicose veins are a common and common pathology, which seriously affects the function of reproduction of men. It is estimated that up to 15-25% of cases of male infertility are primitive and between 75-81% of male infertility is caused by varicose veins.

3. How to recognize varicose veins

  • Early stages: Varicose veins usually have no clinical manifestations. Many patients come to the examination for infertility reasons, then happen to discover that the cause is varicose veins.
  • At a late stage: A common symptom is that the person feels severe sensations, pain, discomfort underneath the testicles. When touched, the veins can be seen zigzagging under the skin. If the testicles are dilated for a long time, the testicles will be smaller than the opposite side. Up to 80-90% of cases of varicose veins in the left testicle.

4. Diagnostic measures

Diagnosis of varicose veins is usually based on two forms:

4.1 Clinical diagnosis

Evaluate the person in many positiones: Lying, standing position. Combine look and touch to assess the level of varicose veins according to Dubin levels:

  • Level 0: Difficult to recognize through clinical signs, identified only through ultrasound, an anethortic or other diagnostic methods.
  • Level 1: Valsava palpation found dilated veins
  • Level 2: When the person is in an upright position palpation of the vein tuft.
  • Level 3: When the person is in an upright position, the varicose vein tuft is seen.
  • Level 4: It is easy to see the tuft of varicose veins zigzagging under the skin of the scrotum whether the person is in a standing or lying position.

4.2 Subclinical diagnosis

  • Ultrasonic Color Doppler

Normally a vein aperture of less than 2mm, when the veins are >2.5mm in diameter, it is diagnosed as dilated. There are 5 levels of varicose veins on Doppler ultrasound:

  • Level 1: No varicose veins in the scrotum but reflux flow of inguinal veins is observed.
  • Level 2: Do not dilate in a lying position but the standing position is found to have dilated and the reflux current is located above the testicles.
  • Level 3: Do not dilate in a lying position but the standing position is dilated and the reflux flow spreads above and below the testicles.
  • Level 4: Varicose veins and reflux currents are seen when valsalva is tested in a lying position.
  • Level 5: See varicose veins, have reflux currents even without testing Valsava.
  • Ultrasound combined with abdominal computer cirrhography (CT)

Necessary tests help to exclude cases of varicose veins of the primary crystal due to post-iton tumors or in the pinched pelvis.

  • Semen

Semen is in place in cases of infertility to assess the extent of the effect of varicose veins on sperm reproduction function.

5. Treatment of varicose veins

The treatment of varicose veins depends on the degree of dilation:

  • At a mild level (level 1.2) it is possible to monitor the combination of drug use to prevent the progress of the disease.
  • At a heavier level (level 3,4,5), when the use of the drug is ineffective may require surgical intervention (open surgery, laparoscopic surgery, microsurgey …).

Currently, with the development of photoe optics, Percutaneous varicocele embolization is being used commonly in Vietnam and gradually replaces the surgical form due to the advantages: minimal intrusive properties, high efficiency, no anesthesia or anesthesia, no scarring. , there is no risk of tightening into the vasectitism.

Materials used: Metal spirals (coils), balls, fiber causing …

Under X-ray instructions, insert the small needle tube (catheter tube) into the right femoral vein and then thread it onto the middle vein and then pass through the left and the selective node and the varicose vein branch.

Vascular node methods for varicose veins have advantages:

  • High aesthetics due to not having to make skin incisions inguinal areas, scrotum
  • No risk of damage, vasecti vasectitis
  • The therapeutic effect is similar to surgery, including local berries and the effectiveness of infertility treatment
  • Health Hubization and recovery time, return to daily activities (<24h)
  • It is possible that the vascular node for both sides with only 1 line into the vein, without sutures of the skin like surgery.
  • Just anesthesia in the general femoral vein area, no general anesthesia or spinal anesthesia is required.

Vascular node for varicose veins is a technique that is important in diagnosing and supporting interventions to treat varicose veins in particular and many other vascular diseases in general. When performing the procedure, the patient should follow the instructions of the doctor to achieve a high therapeutic effect, reducing the risk of accidents.
