Uterus malformation can affect fertility

Mostly, women with uterus malformation do not have difficulty during pregnancy and have normal pregnancy results. However, malformation of the uterus is often detected in women who are infertile and have consecutive miscarriages and premature birth.

1. Types of malformation, abnormalities of the uterus affecting the possibility of pregnancy

Classification of uterus malformation according to ESHRE/ESGE 2013:

  • Degree 0: Normal uterus (Fig. 1: Class Uo/ Normal uterus)
  • Degree I: Abnormal uterus (Fig. 2: Class U1/ Dysmorphic uterus): Ia: T-shaped uterus; Ib: Pediatric uterus; Ic: Other
  • Degree II: Septum uterus (Fig. 3: Class U2/ Septate uterus): IIa: Partly separated uterus; IIb: fully separated uterus
  • Level III: Two-horned uterus (Fig. 4: Class U3/ Biocorporeal uterus): IIIa: Two-horned uterus; IIIb: Fully horned uterus; IIIc: Two-horned uterus with septum.
  • Degree IV: One-horned uterus (Fig. 5: Class U5/ Hemi uterus):: IVa: primitive horns with sinuses; IVb: primitive horns without sinuses.
  • Degree V: Uterus real estate: (Fig. 6: Class U6/ Aplastic uterus): Va: primitive horns with sinuses; Vb: primitive horns without sinuses
  • VI: Unknown

Uterus malformation usually does not show symptoms, in some cases can cause pain in the malformation of the uterus causing blockages. Mostly, women with uterus malformation do not have difficulty during pregnancy and have a normal pregnancy outcome. However, malformation of the uterus is often detected in women who are infertile and have consecutive miscarriages and premature births.

Depending on the classification and extent, malformation of the uterus can cause infertility (degree V), increase the risk of miscarriage of the first and second months, premature birth, delayed pregnancy in the uterus (degrees I, II, III), pre-convulsions and abnormal pregnancies (degrees Ia, II, III).

2. How to detect malformation/ abnormalities in the uterus?

Regular gynecologic examination is the best way to detect genital malformation, especially, if there are abnormal symptoms such as: Late puberty,dysolic pain, iisto kinh,infertility, consecutive miscarriages….

On clinical examination and ultrasound can detect uterus malformation,3D ultrasound is used to determine the diagnosis. When complex uterus malformation is suspected, the diagnosis is determined by an MRI scan that combines laparoscopy and endoscopy of the uterus.

When detecting malformation/ abnormalitiesin the uterus, depending on the degree, the doctor will advise specifically on the risk and plan of pregnancy as well as monitor the pregnancy. Surgical treatment is recommended only for women who have consecutive miscarriages and septic uterus malformation. In this case, surgery can increase the chances of successful pregnancy despite the risks in surgery, especially leaving the uterus scarring.
