Narrowing, coronary artery obstruction: Who should put the stent? Who should have surgery?

Coronary arteries are a blood supply system extremely important for the heart in particular and for the entire human body in general. Let's learn about coronary artery diseases and offer the most appropriate treatment options.

1. What is coronary artery and coronary artery pathology

Coronary arteries are vascular systems that nourish the heart. Each of our hearts has two main coronary arteries: the right coronary artery and the left coronary artery. These coronary arteries come from the base of the aortic and run on the surface of the heart. The left coronary artery runs a short distance, called the common body of the coronary artery (1 – 3cm) then divided into 2 large branches: the front aortic artery and the exponential artery.

Thus, the coronary artery system has three large branches that nourish the heart: the front aerial artery, the cap artery and the right coronary artery. These three large branches give off a lot of smaller arterial branches such as wall branches, diagonal branches, shore branches … will be responsible for carrying oxygen-rich blood from the aortic to nourish all the structures in the heart.

When coronary artery disease means that the blood flow from the coronary artery to the heart muscle decreases, then the heart muscle does not receive enough oxygen and angina appears.

2. Coronary artery stenosis

Coronary artery stenosis is a condition that appears when one or more branches of the coronary artery are narrowed or hindered by the formation of atherosclerosis plaques attached to the inside of the vessel. This is because the human body degenerates with age, causing the coronary arteries to no longer be soft and elastic, or an unscscular lifestyle that causes the blood circulation system to be affected, resulting in the coronary artery being affected accordingly. Due to anemia that nourishes the heart, the heart muscle cannot receive the necessary amount of blood and oxygen, causing angina or myocardial infarction and entails extremely dangerous complications.

In addition, with the worsening of the course over time, coronary artery disease causes myocardial anemia, which gradually weakens, leading to heart failure and arrhythmias . It can be seen that these are extremely dangerous diseases for the health and life of each person so patients need to have adequate understanding to detect as well as have the most effective treatments.

Doctors often give advice on two options: coronary artery nod and stenting, or coronary artery brimming surgery. These are all the most modern treatments of medicine, but to choose which option is suitable for each patient is something to consider carefully.

3. Coronary dilatation and stenting

This is called a periocularcoronary artery intervention technique, in which doctors will use a small catheter to insert a small shadow into the clogged coronary artery and then nod and place the Stent to re-open the blood flow.

Coronary artery intervention can be done only by opening a small hole in the skin to insert the catheter into the artery in the thigh or wrist. The procedure is usually carried out within 1 hour and the majority of patients can go home after 1-2 days from the end of the procedure.

However, this method is only suitable for patients with coronary artery stenosis not too severe and due to the simplicity and convenience of the technique. However, for patients with severe and complex coronary artery stenosis, the better option is coronary artery bridging surgery.

4. Coronary artery bypass grafting

Coronary brising surgery is performed using a healthy blood vessel taken from parts such as the legs, arms, back, abdomen to make the artery connect the bridge from the aortic through the blocked coronary artery, helping to create a new path that leads blood to feed the heart muscle.

However, with the surgical method, doctors must always consider based on the patient's health status and are usually applied to patients with severe coronary artery stenosis, complexity or having had angioplasty, stent placement but unsuccessfully, or then with the phenomenon of vascular stenosis again.
